2023 Dog Registration Application

Personal information contained on this form is collected under the authority of the Municipal Act, 2001, S.O. 2001, c.25 and will be used for the purpose of administering the Municipality of Lakeshore's dog licensing program.

All dog owners are required to purchase a dog licence every year for each dog in their possession. All licences expire on December 31st of the year issued.

The dog licence is $22.00 per dog. Purchases of dog licences made after March 31st each year are subject to a $10.00 late fee.

Replacement tags are available for $6.50.  There are no refunds on dog tags.

1. I consent to the collection of my personal information included on this form, for the purpose of administering the Municipality of Lakeshore's dog licensing program.
2. I confirm that the dog that I am registering is NOT any of the following:
   1. Any dog of the Pit Bull, Staffordshire, Bull Terrier, American Pit Bull, American Staffordshire Terrier or Presa Canario breeds of dog;
   2. Any hybrid dog produced from a breed mentioned in (a) above;
   3. Any crossbreed dogs that possess substantial characteristics of one or more of the dog breeds referred to in (a) above.
